E-Verify currently experiencing intermittent errors

The Department of Homeland Security has acknowledged that E-Verify users are experiencing issues in which cases are returning "duplicate case" eligibility statuses for submissions that are not true duplicates.

United States Citizenship and Immigration Services (USCIS) is currently working to fix the issue.
