New disclosure and authorization form required to request Georgia statewide criminal history searches

Effective Friday, March 22, 2019, Georgia statewide criminal history searches will require the subject of the search to complete a new name-based criminal background history record information consent/inquiry form.

Copies of the new form are available below.

This new requirement is being enforced by the record source with short notice. It is strongly recommended to start utilizing this new form as soon as possible.
