Notice regarding expiration of Old CCF

The 2014 Federal CCF (Custody and Control Form), referred to as the Old CCF, expires on June 30, 2018.

If a testing laboratory receives specimen with the Old CCF after June 30, 2018, it will request an MFR (Memorandum for the Record) from the collector to explain the use of the incorrect form. The laboratory is required to hold the result for five business days while attempting to obtain an MFR. If a lab cannot obtain an MFR within five days, the specimen will be reported as "Rejected for Testing."

Hence, using the Old CCF can result in delays and increased turnaround times for drug test results. Therefore, clients should ensure that they discard any Old CCFs and are using only the new 2017 forms.
